I’m Keri Andrews, President and founder of Life Transitions Case Management, where my passion is helping adults with intellectual disabilities thrive beyond high school.
My Story
With over 30 years of experience, I’ve dedicated my career to bridging the gap in services that so often leaves families feeling uncertain about the future.

My expertise lies in providing personalized case management, life coaching, and job coaching services. From guiding individuals toward independent living to securing access to healthcare and benefits, I approach each challenge with strategic planning, compassionate care, and a commitment to problem-solving.
My career has spanned leadership roles, including Director of Administration at Lamb’s Farm, where I spearheaded community outreach and program improvements. Founding Life Transitions Case Management allowed me to focus on what I do best—empowering individuals to achieve their fullest potential and supporting families through complex transitions.
I hold a Bachelor of Science in Psychology from Northern Illinois University and am a Qualified Intellectual Disabilities Professional (QIDP). With both knowledge and heart, I’m here to help guide you or your loved one toward greater independence and success.
