Throughout my career, I have encountered numerous parents who find themselves unprepared and uninformed when their disabled child completes their high school education. This often leaves families struggling to navigate the complexities of the social service system. To address this pressing issue, Life Transitions Case Management provides comprehensive services that ensure a seamless transition to independence for all parties involved.

Case Management

One-on-one case management, tailored to meet the unique needs of each client and their family.

Identifying information deficits and providing families with vital insights into SSI, SSDI, Medicare, Medicaid, social service programs, and PUNS.

Collaborating with the support team to secure the best possible outcome for students transitioning to the adult system.

Offering education and guidance on state and federal adult programs available to the client.

Providing expert assistance with the completion and filing of necessary paperwork with the State of Illinois.

Career Coaching

Exploring college and vocational opportunities in partnership with the client and their family.

Life Coaching

Cultivating essential life skills, such as laundry, basic cooking, job application, job coaching, and money management, to foster independence.

Facilitating the enhancement of social skills by connecting individuals to local activities and, when possible, organizing social skills groups in the Lake county community.
